What does hand hygiene actually remove?

Hand hygiene is a mechanical action before it is a chemical one. Soap lifts oils, dirt and the microbial load that sits on skin, and running water carries the suspension away. Alcohol based rubs work differently: at 60 to 80 percent ethanol or isopropanol they denature proteins in bacteria and enveloped viruses, which is why they are fast but less effective on spores and on hands visibly soiled.

The World Health Organization describes five moments in care settings, before touching a patient, before a clean procedure, after body fluid exposure, after touching a patient, after touching surroundings. Outside care settings the useful version is shorter: before eating, after using the toilet, after handling raw meat, after coughing or sneezing, after contact with a sick person. How do vaccines work?

A vaccine presents the immune system with a harmless version of a pathogen, or with instructions to build one protein from it, so that the response is learned before the real exposure. The adaptive arm has two outputs. B cells produce antibodies that bind the target; T cells recognise infected cells and coordinate the response. After vaccination, a fraction of those cells persists as memory, which shortens the delay on later contact.

Types differ in how the antigen is delivered. Live attenuated vaccines use a weakened organism and tend to produce long lasting responses with one or two doses. Inactivated vaccines use killed organisms and often need adjuvants and repeat doses. Subunit and mRNA vaccines deliver a specific protein or the recipe for it, which allows faster updating when a virus changes.

Reactogenicity, sore arm, low fever, fatigue, is the expected consequence of immune activation and usually resolves in one to three days. Population effects depend on coverage: when enough people are immune, transmission chains break and those who cannot be vaccinated are indirectly protected. Which periodic checks are worth scheduling?

Periodic checks are a screening question, not a general check up question. A test earns its place when it detects a condition early, when early treatment changes the outcome, and when the test is accurate enough that false results do not cause more harm than the disease.

Blood pressure is measured at every adult visit because it is cheap, silent and treatable. Lipid profile and fasting glucose are typically checked every few years in adults at average risk, more often when a previous value was borderline. Colorectal screening follows age bands, often starting at 50 and sometimes at 45, with stool tests or colonoscopy at intervals set by national programmes. Cervical screening uses cytology or HPV testing on a schedule, and breast screening uses mammography in defined age ranges. Dental checks, eye examinations and skin checks are added according to risk factors rather than to a universal calendar.

What the numbers mean matters as much as the test. A single elevated reading is a signal to repeat, not a diagnosis. Trends across years carry more information than one value, and the reference range printed on a report is a population interval, not a personal target.

What can a food label tell you?

A food label is a structured document with four zones: ingredients, nutrition declaration, quantity of specific ingredients, and storage or preparation instructions. Ingredients are listed by descending weight, so the first two or three entries describe what the product mostly is. A cereal whose first ingredient is sugar is a different object from one whose first ingredient is whole grain, whatever the front of the pack says.

The nutrition declaration is usually given per 100 grams and per portion. The per 100 grams column is the one that allows comparison between products, because portion sizes are chosen by the manufacturer. Reference intakes, often printed as percentages, are benchmarks for an average adult, not requirements. Salt is listed as salt, and the conversion from sodium is roughly 2.5 to 1, so 0.5 grams of sodium equals about 1.25 grams of salt.

Fibre, protein and fat quality deserve attention alongside energy. A product can be low in calories and low in fibre, which makes it filling for a short time. Front of pack schemes vary by country and are simplified summaries; the back of pack panel is the primary source.

What do the numbers for sleep and weekly movement mean?

Sleep recommendations for adults sit at seven to nine hours per night, with older adults often at seven to eight. Duration is the headline figure, but regularity is a second variable: going to bed and waking at consistent times stabilises the circadian signal. Sleep debt accumulates, and recovery sleep on weekends does not fully cancel it. Falling asleep within 15 to 30 minutes and waking without an alarm are ordinary markers of adequate duration.

Weekly movement is expressed in two currencies. Aerobic activity is counted in minutes at moderate intensity, 150 to 300 per week, or 75 to 150 at vigorous intensity, and the two can be combined. Muscle strengthening is counted in sessions, two or more per week covering major groups. Moderate intensity is often defined as a level at which you can talk but not sing. The 150 minute figure can be split into five sessions of 30 minutes, or into short blocks across the day, because the total weekly volume is what the evidence tracks.

How are blood pressure and blood sugar read?

Blood pressure is written as two numbers, systolic over diastolic, in millimetres of mercury. For most adults the threshold for hypertension is 140 over 90 in the clinic, with 130 over 80 used in some guidelines and for home measurements. Home readings tend to run a few points lower than clinic readings, which is why the two are not interchangeable. A single high value is repeated on another day before any conclusion is drawn.

Blood sugar is read in milligrams per decilitre or in millimoles per litre, depending on the country. Fasting glucose between 70 and 99 mg/dL is usually considered normal, 100 to 125 mg/dL is impaired fasting glucose, and 126 mg/dL or above on two occasions supports a diabetes diagnosis. HbA1c, the percentage of glycated haemoglobin, reflects average glucose over roughly three months, with 5.7 to 6.4 percent as the intermediate band in the American classification. Post meal values peak around one to two hours and are interpreted against the timing of the measurement.

The common thread across these figures is that they are distributions with cut offs, not pass or fail marks. A value near a boundary is a reason to repeat the measurement, review the context and look at the trend, which is the same logic that applies to hand hygiene, vaccine schedules and food labels: a defined action, a defined interval, a documented result.
